I authorize Shapes Fitness for Women, AZO Fitness, Inc. ("Franchisee"), and Shapes Franchising, LLC ("Franchisor") and each of their agents, representatives, and affiliates, to contact me via telephone call and text message at the telephone number that has been provided for sales and marketing purposes using live operators, automated telephone dialing systems, or prerecorded messages. Message and data rates may apply. I am not required to consent as a condition of any purchase.
I agree that by my providing my phone number(s) and/or email address(es) to us: Franchisee, Franchisor and each of their affiliates, franchisees, agents, employees, officers directors and representatives may contact the e-mail address(es) and phone number(s) I provide to Franchisee or Franchisor to provide me with information, offers, new product or service offerings, or otherwise communicate with me.
Notwithstanding any current or prior election to opt in or opt out of receiving telemarketing calls or SMS messages (including text messages) from Franchisee, Franchisor or their agents, franchisees, representatives, employees, officer, affiliates, or anyone calling, texting or e-mailing on their behalf, I expressly consent to be contacted by Franchisee, Franchisor, or their agents, franchises, representatives, affiliates, employees, officers, or anyone calling on their behalf for any and all purposes arising out of or relating to my current, former or potential membership and/or any other products or service offers or purchases from them, at any telephone number, or physical or electronic address i provide or at which I may be reached.
I agree Franchisee, Franchisor and each of their, agents, representatives, employees, officer, affiliates and franchises may contact me in any way, including SMS messages (including text messages), calls using prerecorded messages or artificial voice, and calls and messages delivered using auto telephone dialing system or an automatic texting system. Automated messages may be played when the telephone is answered, whether by me or someone else.
In the event that Franchisor, Franchisee, an agent or representative, employee, officer, affiliate or franchisee calls, the caller may also leave a message on ymy answering machine, voice mail, or send one via text.
I consent to receive SMS messages (including text messages), calls and messages (including prerecorded and artificial voice and autodialed) from Franchisor, Franchisee, and their, agents, employees, officers, representatives, franchisees affiliates or anyone calling on their behalf at the specific number(s) I have provided to any of them, or numbers they can reasonably associate with my account (through skip trace, caller ID capture or other means), with information or questions about offers from them, my membership application and/or membership, or other product or service purchases from any of them.
I certify, warrant and represent that the telephone numbers that I have provided to them are my contact numbers. I represent that they are permitted to receive calls at each of the telephone numbers I have provided to them. I agree to promptly alert them (and the sender if different) whenever I stop using a particular telephone number. My cellular or mobile telephone provider will charge my according to the type of plan I carry.
I also agree that Franchisee, Franchisor, and their representatives, agents, officer, employees affiliates and franchises may contact me by e-mail, using any email address i have provided to any of them or that I provide to any of them in the future.
I agree that Franchisor, Franchisee, as well as their, employees, officers, agents, representatives, affiliates and franchises may listen to and record calls for quality monitoring purposes.
